(3.1) DPH-Community Health Epidemiology and Disease Control. Presentation of a Resolution Approving an Agreement with Santa Clara County for Membership in the Bay Area Regional Immunization Registry.
*Joint Powers Agreement(3.2) AIDS Office-HIV Health Services - Request for approval for contract modifications to
extend the term for an additional year eight FY 2003-2004 Ryan White Comprehensive AIDS Resources Emergency (CARE) Act Title I, contracts for total of $2,796,023, for the period of March 1, 2004 through February 28, 2005. The two year combined total for the period of March 1, 2003 through February 28, 2005 for these contracts will be $5,592,046 with the following agencies:

(3.3) AIDS Office-HIV Prevention - Request for approval to accept and expend a grant from the Centers for Disease Control and Prevention, in the amount of $393,478, to design and implement a demonstration project to use rapid HIV tests to improve outcome of partner counseling and referral services, for the period of September 15, 2003 through September 14, 2005.
* Calendaring request memo, Grant information form, Budget pages(3.4) AIDS Office-HIV/AIDS Statistics and Epidemiology - Request for approval to accept retroactively and expend a three-year grant from the Centers for Disease Control and Prevention, in the amount of $518,646, for a study of HIV risk behavior surveillance among men who have sex with men, for the period of September 30, 2003 through September 29, 2006, and a sole source contract with UCSF Center for AIDS Prevention Studies, in the amount of $291,723 for the same time period.

* Calendaring request memo(3.8) CHPP-Health Promotion - Request for approval of a retroactive contract renewal with Polaris Research and Development, in the amount of $400,000, to provide Black Infant Health Improvement Program services targeting African American parents, for the period of July 1, 2003 through June 30, 2004.

* Calendaring request memo, Client demographic information form(3.11) CHP-Housing and Urban Health - Request for approval of a contract renewal with Baker Places, Inc., for the Behavioral Health Demonstration Project, in the amount of $397,725, to provide behavioral health services targeting Direct Access to Housing participants, for the period of September 30, 2003 through September 29, 2004.
* Calendaring request memo, Client demographic information form(3.12) CHP-Housing and Urban Health - Request for approval of a retroactive new contract with Chinatown Community Development Center, the lead agency and fiscal agent for the SRO Families Collaborative, in the amount of $100,000, for the period of July 1, 2003 through November 30, 2003.
* Calendaring request memo(3.13) CBHS-Mental Health - Request for approval of a retroactive contract modification with Bay Area Bookkeeping, in the amount of $270,000, for fiscal year 2003-04 only, for check-writing fees and payments to State-mandated non-contract hospital providers, for a total amount of $1,195,120 for fiscal year 2003/04, and a total contract amount of $3,970,480 for the period of July 1, 2003 through June 30, 2007.

St. Joseph-St. Elizabeth’s Epiphany Center for Families in Recovery, in the amount of $1,074,666 per year for a total contract value of $4,298,664, to provide residential and outpatient substance abuse services, for the period of July 1, 2003 through June 30, 2007.
