(3.1) BHS-Substance Abuse – Request for approval of retroactive contract renewal with Addiction Research and Treatment, Inc. dba BAART, in the amount of $5,740,202, to provide substance abuse treatment services, for the period of July 1, 2004 through June 30, 2005.
* Contract summary, Monitoring report summaries, Client demographic information forms(3.2) BHS-Substance Abuse – Request for approval of a retroactive renewal contract with Curry Senior Center, in the amount of $510,188, to provide substance abuse treatment and mental health services, for the period of July 1, 2004 through June 30, 2005.
* Contract summary, Monitoring report summaries, Client demographic information forms(3.3) BHS-Substance Abuse – Request for approval of a retroactive renewal contract with Westside Community Mental Health Center, Inc., in the amount of $2,850,755, to provide substance abuse treatment services, for the period of July 1, 2004 through June 30, 2005.
