WHEREAS, the Health Department's patient care sites and providers have been reorganized into a single, integrated delivery system known as the Community Health Network of San Francisco; and,
WHEREAS, San Francisco General Hospital and Laguna Honda Hospital have had long-standing Pharmacy and Therapeutics Medical Staff Committees which implemented hospital formularies focused on cost-effective clinical practices; and,
WHEREAS, the Community Health Network needs a uniform formulary for its patient care sites and providers; and,
WHEREAS, the Community Health Network established a Pharmacy Council charged with formulating a procedure for evaluating, approving, and communicating new pharmaceutical policies and practices throughout the Community Health Network; and,
WHEREAS, this Pharmacy Council of the Community Health Network Providers recommends that the Medi-Cal formulary be used as the basis for the Community Health Network Formulary; and,
WHEREAS, this Pharmacy Council of providers further recommends that other essential drugs needed on the formulary be evaluated based on clinical necessity, cost effectiveness, compliance advantage, and standard of care; and,
WHEREAS, this Community Health Network Council further recommends that all drugs not on the formulary require prior authorization from the payor or its designee; now, therefore, be it
RESOLVED, that the Health Commission hereby approves the application of a single Community Health Network Formulary and formulary process for all Medi-Cal and County sponsored patients.
