Resolution:

Authorizing Approval of Revision of the San Francisco Adult Day Health Care County Plan, April 7, 1998

WHEREAS, the State statutes and regulations require Counties to amend the Adult Day Health Care County Plan whenever service area boundaries need to be changed as recommended and approved by the local Adult Day Health Care Planning Council; and,

WHEREAS, the local Adult Day Health Care Planning Council is appointed by the County Board of Supervisors and approved by the Department of Aging to plan for organized day programs of therapeutic, social and health activities and services provided to the elderly and persons with functional impairments either physical or mental, for the purpose of restoring or maintaining optimal capacity for self care; and,

WHEREAS, the San Francisco Adult Day Health Care County Plan was revised to: 

  1. update all State-required demographic data;  
  2. allow multiple providers in a single service area, if there is a demonstrated need; and,  
  3. expand Service Area #4, develop new Service Area #5 which was part of former Service Area #7, expand Service Area #6, move Service Area #7 to include part of former Service Area #5, and expand Service Area #8; and,

WHEREAS, these revisions were made in accordance with State mandated processes and formalized by the Adult Day Health Care Planning Council on March 11, 1998, now, therefore, be it

RESOLVED, that the Health Commission supports the revised San Francisco Adult Day Health Care County Plan, and, be it

FURTHER RESOLVED, that the Health Commission recommends these revisions to the Board of Supervisors for approval and submission to the State Department of Aging.
