RFP 1-2019The San Francisco Department of Public Health is soliciting proposals for a Vendor-Hosted Software Interagency Care Management Solution. A detailed description of the work can be found in Section II of the RFP. The description of work listed in Section II is a general guide and not a complete description of all aspects of the work or of all tasks necessary in order to complete the work.
