RFP 13-2021  The San Francisco department of public health is soliciting proposals for THIRD PARTY ADMINISTRATOR FOR 340B CONTRACT SPECIALTY PHARMACY NETWORK ADMINISTRATION SERVICES FOR THE DEPARTMENT OF PUBLIC HEALTH. The description of work listed in the RFP document is a general guide and not a complete description of all aspects of the work or of all tasks necessary in order to complete the work.

PLEASE NOTE:

  1. Proposers that previously submitted a Letter of Intent do not need to submit a new Letter of Intent as a result of this amendment.

  2. All previously submitted E-Questions will be dis-regarded and all proposers will need to resubmit E-Questions under the new E-Question session which begins 8/11/2021.

  3. Proposers will need to re-download the RFP package to see all updates and changes. Changes have also been made to the following forms in Attachment 1: Letter of Intent.pdf, Minimum Qualifications.pdf, and Customer Certifications.pdf.
