Off-Site Storage and Destruction of Records and X-Rays Services Medical Record Maintenance

RFP 35-2019 The Department of Public Health is seeking proposals for timely and reliable off-site storage and destruction services, for the San Francisco Health Network (SFHN), Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG), Laguna Honda Hospital and Rehabilitation Center (LHH) and Jail Health Services for protected health information (PHI) of patient’s medical records as defined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and final Administrative Simplification Regulations as well as other documents which must be retained for defined periods of time. The vendor awarded a contract must be compliant with the Business Associates provisions of HIPAA of the Federal government of the United States of American and its Administrative Simplification regulations.

The Scope of Work is used as a general guide and is not intended to be a complete list of all work necessary to complete the project.
