Professional Services in Conduction Patient and Workforce Experience Surveys

RFQ 15-2020 The San Francisco Department of Public Health (SFDPH) is seeking submissions from qualified vendors/contractors to serve as designated vendor for the collection of standardized patient and staff experience and provider engagement survey data. SFDPH aims to collect and publicly report standardized patient experience survey data about the healthcare experience in the SFDPH. The department aims to use a standard survey instrument and approach to assess, compare and publicly report patient experience survey data at the practice-site level. Whether an applicant has, such qualifications will be determined through the evaluation and selection criteria described in Section IV of this Request for Qualifications (RFQ). No applicant shall have any legal or equitable right or obligation to enter into the contract or to perform the work as a result of such application being chosen.
